Hartford Nursing & Rehabilitation Center Receives Citation for Resident Elopement

On May 1, 2023, Hartford Nursing & Rehabilitation Center received a citation for failing to ensure that the nursing home area was free from accident hazards and failing to provides adequate supervision to prevent accidents.

The citation related to an incident on April 6, 2023, when the nursing home failed to adequately supervise a resident. As a result around 10:16 pm, the resident left the facility through the front door without the knowledge of the facility staff. The facility staff was unaware of the resident’s disappearance until April 7, 2023 at 7:50 am, which was approximately 10 hours later. The resident was found by local police on 6 Mile Road and Telegraph Road, approximately five miles away from the facility. Local police took the resident to the local hospital on April 7, 2023 at 3:08 am.

This state determined that allowing a resident to leave the facility unsupervised and without staff knowledge was an immediate jeopardy. Immediate jeopardy is a situation in which the nursing home’s non-compliance with one or more requirements has caused, or is likely to cause, serious injury, harm, impairment, or death to a resident.
